As a benefit of membership, MSD offers members the option of utilizing the MSD CME Transcript Service to assist in keeping track of all Category 1 Credits to ensure compliance with state licensing requirements. This service is available to those members who subscribe.

ONLY THOSE MSD MEMBERS WHO HAVE SUBSCRIBED TO THIS SERVICE WILL BE RECORDED. If you had previously completed the form, your subscription is on-going.

MSD accredited activities are automatically recorded to your transcript. For activities that are NOT sponsored by MSD, adequate documentation of participation (such as copies of certificates, letters from event sponsors, attendance sheets, etc.) must be submitted by email, mail or by fax to the attention of the Office of Professional and Public Education. Please keep copies of all documentation sent to MSD.
